Martin Luther Campus offers short-term, transitional care for adults who need rehabilitation services after an illness or injury. Designed to bridge the gap between hospital care and returning home, our Transitional Care program crafts a rehabilitation plan to meet a patient’s specific goals and needs. We help patients with complex conditions transition back to their homes, or the next stage of care.
Our highly trained, licensed nursing staff works with you or your loved one’s health care provider to develop a personalized care plan designed to maximize the resident’s independence, so they can successfully transition back into their normal life.
